The forms provided below are to help expedite your first visit at Aesthetic Specialty Centre. These four forms will be required to create your medical file. Please download, print the forms, fill out the information as accurately as possible and bring with you to your appointment. All information provided is kept completely confidential.

Pre-Operative Care

The forms provided below are to assist you in preparing for your surgery. The information provided will be covered in detail at your pre-operative teaching appointment.

Post-Operative Care

The forms provided below are to give you an overview of what to expect and how to care for yourself during your recovery period. These forms will be reviewed with you in detail at your pre-operative teaching appointment.

Privacy Policy

It is the policy of our practice that all physicians and staff preserve the integrity and the confidentiality of protected health information (PHI) pertaining to our patients. The purpose of this policy is to ensure that our practice and its physicians and staff have the necessary medical and PHI to provide the highest quality medical care possible while protecting the confidentiality of the PHI of our patients to the highest degree possible. Patients should not be afraid to provide information to our practice and its physicians and staff for purposes of treatment, payment and healthcare operations (TPO).
